Dear Applicant

As part of the credentialing process for Pitt County Memorial Hospital, all physicians, physician extenders or other personnel having direct contact with patients must comply with the Medical Staff Occupational Health Policy. Before you are granted the right to provide patient care, the following must be documented:

  1. PPD skin test
  2. Immunity to the following viral infections,: measles, mumps, rubella, and varicella

It is highly recommended that you obtain all documentation of immunity from either your personal physician or your occupational health office at your current institution prior to arrival. However if tour are unable to accomplish this prior to arrival, physician and physician extenders being employed by the east Carolina University School of Medicine can receive all occupational health services at the ECU Office of Prospective Health upon arrival. Private practitioners can utilize the physician of their choice or the ECU Office of Prospective Health.

Documentation Required

  1. A PPD Skin Test must be placed and read before credentialing and yearly thereafter for recredentialing (for those with negative tests). You may read your own test and place the results on the linked form. If the result is positive (>10 mm induration), then a chest x-ray must be obtained and treatment for latent tuberculosis (formerly known as prophylaxis) considered. If you are known to have a positive PPD skin test, than a recent chest x-ray report (within 2 years) and the medical record of receiving treatment for latent tuberculosis is required.

For the viral illnesses, the following must be documented with medical records:

  1. Measles and mumps: physician diagnosis of illness or MMR vaccine or positive antibody titer (only for persons born on or after January 1, 1957)
  2. Rubella: MMR vaccine or positive antibody titer
  3. Varicella: history of chicken pox or varicella vaccine or positive antibody titer
